THIS CANCELLATION FORM IS ONLY VALID FOR THE FITNESS 2000 KING, NORTH CAROLINA LOCATION!
Once your membership agreement has been fulfilled you may cancel by following the instructions below:
Your membership agreement requires a 30 Days Written Notice from postmarked date. You are responsible for all fees until your cancellation is effective. You will continue to be billed until your cancellation has been approved and processed. Cancellations will NOT be processed if your membership account has a balance. Click below to download and print form. Once completed, this form must be mailed to address below.
If you are moving at least thirty (30) miles from our location and eight (8) miles from your current address, you may cancel at any time with a 30-day written notice. Print and fill out the Membership Cancellation Request Form and send it to the address below. Proof of Move must also be included. Examples of a Proof of Move include: signed lease, utility installation order or bill, etc. It must also contain Member’s name and previous address.
